By signing our consent form you acknowledge and authorise Southside Physio to obtain or supply, to or from, any other referring medical practitioner or health care provider, relevant personal and medical information regarding your current condition (i.e. the reason for attending Southside Physio) or that of your child or person under your legal care. Where disclosure of personal or health information to a third party that would not reasonably expect to be required is necessary, we will seek your consent prior to the disclosure.

On occasion, your therapist may use AI tools such as Heidi to assist with administrative tasks such as note-taking. At your first session, your therapist will seek your verbal consent before using AI in this way. Once consent is provided, it will apply to subsequent sessions without the need for repeated requests.

These tools may be used to transcribe parts of the session, suggest clinical notes, or generate supporting documents such as referral letters. By signing the Privacy and Disclosure of Medical Information Authority, you are consenting to the potential use of these AI tools.
