CHRONIC DISEASE MANAGMENT PLAN (CDM) 

– PREVIOUSLY CALLED AN ENHANCED PRIMARY CARE PLAN (EPC) 

A Chronic Disease Management Plan is a Care Plan organised through a GP to help people who have chronic conditions.  A chronic condition is a medical condition that is likely to be present for 6 months or longer, a terminal condition or complex needs. 

The patient is able to have up to 5 (total) allied health sessions per calendar year – 1st January to 31 December.  E.g.   Physiotherapy, Exercise Physiology, Dietitian etc.  If you are an Aboriginal or Torres Strait Islander, 10 sessions are available.  

The Care Plan can be issued at any time of the year, and the sessions can be used anytime within the referral period.  If you have unused sessions at 31 December, you can still use them for the next year, however they will come out of that calendar year’s allocated sessions.  

It is important that you let Reception know prior to your appointment that you have a Chronic Disease Management Plan/Enhanced Primary Care Plan so that we can use the correct coding and have all of your details ready to go. 

How is the Rebate given?  – We ask that you pay for the full amount of your consultation straight after your appointment.    We can then process your Medicare Rebate on the same day via our HICAPS machine IF you have an EFTPOS card attached to a savings or cheque account. The Medicare Rebate will then go back on to your EFTPOS savings/cheque card.     

(Medicare will not allow us to provide rebates to your credit card account.)   

 If you don’t have an EFTPOS card attached to a saving or cheque account, we can issue you with a receipt for you to claim your rebate directly with Medicare either in person or online.   

The current Medicare Rebate is $60.35 per consultation, regardless of the specific service that you choose.  That means that the rebate is the same for the initial consultation and any subsequent review consultations that you might be recommended for as well. 

For us to be able to claim your Medicare Rebate, you will need to bring with you: 

  • A copy of the CDM/EPC Care Plan 
  • A physical Medicare Card
  • A physical EFTPOS –savings/cheque card (make sure you know your PIN)
